Death Certificate Requests via Mail WARNING: I.C. 16-1-9-6 - False application, alteration, mutilating, or counterfeiting Indiana death certificates is a criminal offense. Death certificates may be obtained by a spouse, child, grandparent, executor/executrix, and the attorney for an estate of the deceased. One may make application for a death certificate in person, by mail, or by fax. Payment for certificates may be made with cash, check, or money order. Some restrictions may apply, call (317) 346-4372 for additional information. All persons born in Johnson County after 1882 have written birth & death certificates on file with the Health Department. These records are legal copies which can be used for all requests. To obtain a copy of a death certificate, please complete the following: 2. Enclose a copy of your photo identification (ie. Drivers License) 3. Cash, or Money Order, accepted Regular size - $12.00 per certified copy 4. Send a self -addressed stamped return envelope
