Children’s Vaccines Children’s Vaccines available for those 6 weeks to 18 years of age. You may click on each of the vaccines to view a Vaccine Information Statement. We give child immunizations through the state VFC (Vaccine’s For Children) program. We follow the guidelines that the states/CDC sets for us. To be eligible for VFC vaccine, the child must fall into certain insurance guidelines. Children that do not fall into these guidelines will be referred to their doctor. Please call our immunization coordinator to see if your child is eligible for VFC vaccine. Diptheria, Tetanus, Pertussis (DTaP) Haemophilus Influenzae Type B (Hib) Tetanus, Diptheria, Pertussis (Tdap) Human Papilloma Virus (HPV) Gardasil Human Papilloma Virus (HPV) Cervarix Some of the above vaccines are available in combination vaccines. Starting January 1, 2011, there will be a $5 administration fee per visit for 1 child, and $10 per visit for more than 1 child receiving vaccine. (The maximum amount per family, per visit is $10.) Immunizations are by appointment only. You must provide a complete shot record prior to making your appointment. You may call 346-4368 to speak with the immunization coordinator. |
