Birth Certificate Application

WARNING: I.C. 16-1-9-6 - False application, alteration, mutilating, or counterfeiting Indiana birth certificates is a criminal offense.

Certified birth certificates may be obtained by a parent, sibling, or grandparent.  One may apply for a certificate in the office, by mail, or by fax.  Payment for certificates may be made with cash, money order, credit (Visa or Master) or debit card.  We no longer accept out-of-state check.  Some restrictions may apply, call 317)736-3775 for more information.

Birth certificate via mailed application.

All persons born in Johnson County after 1882 have written birth & death certificates on file with the Health Department.  These records are legal copies, which can be used for all requests, including school registration.  If you need a copy of these documents, please contact the Health Department office at your earliest convenience.  Request for these records are heavy during mid to late August.  To obtain a copy of a birth certificate, please complete the following:

1.  Complete Application

2.  Enclose a copy of your photo identification  (i.e. Drivers License)

3.  Cash, Money Order or Visa/Master cards accepted

                - No Personal Checks or out-of-state checks

        Regular size - $10.00 per certified copy

         Combo (1 regular size + 1 wallet size) - $16.00

4.  Send a self-addressed stamped return envelope

 

For records of birth/death outside of Johnson County please contact the Indiana State Department of Health.  You may call the State Department at (317) 233-2700.

Making application by fax, a credit card must be used as payment.  A phone call to (317)736-3775 will begin the application process via fax.  Some restrictions may apply; call 317)346-4367 for additional information.
